St. Joseph’s Healthcare Hamilton’s Youth Wellness Centre’s Transition Support Stream is designed to assist youth who are already receiving intensive or specialized mental health services in the child/youth mental health system and will likely need ongoing services past their 18th birthday. In Ontario, a person who younger than 18 can receive specialized mental health services for “children and adolescents”. When they turn 18, they need to start using adult services. Our team can support youth transitioning from child and adolescent services to adult services that are best suited for their needs. These youth will be connected to our Transition Coach. The Transition Coach can work with the young person to help them get connected to services and treatment that meets your mental health goals, help them prepare for appointments, and support good communication between the young person, their supports, and service providers.
This stream accepts referrals from our community partners:
- Child and Adolescent Services
- Contact Hamilton
- Lynwood Charlton Centre
- McMaster Children’s Hospital
We also accept referrals from similar agencies outside of Hamilton working with youth who are transitioning to adult mental health and addictions services in Hamilton.
