We offer two streams of clinical mental health care:
Early Intervention: This stream serves youth who are experiencing new or emerging mental health or substance use concerns, or those who have not previously sought help for their mental health or substance use.
Transition Support Services: This stream is designed to assist youth who are already receiving intensive or specialized mental health services in the child/youth mental health system and will likely need ongoing services past their 18th birthday. Our community partners (McMaster Child & Youth Mental Health Program, Child & Adolescent Services, or Lynwood Charlton) can refer youth who would benefit from help during the transition to adult services.
What if the Youth Wellness Centre is not the best place for the young person I am referring to receive mental health or addiction care?
Sometimes, the Youth Wellness Centre isn’t the right fit for the young person you’re concerned about, and that’s okay! Our team will still do their best in helping to connect the young person with the service(s) that are right for them. We’ll work with the young person and follow up to help them get the care they require.
The Youth Wellness Centre is part of St. Joseph’s Healthcare Hamilton, the regional lead in mental health care. We’ll help clients navigate the mental health services available by connecting them with one of our community partners, or refer them directly to another program at St. Joseph’s Healthcare Hamilton.

The mental health professionals at the Youth Wellness Centre will carefully review the referral and will connect with your client within two business days.
If our clinical care is a good fit, we’ll book an appointment.
